David did everything wrong.
Cyberpsychosis, in my opinion, isn’t so much different from a normal psyschosis, which is simply a state of psychological disconnection due to trauma. This trauma can itself be physical or psychological. In the case of physical, it can be caused by immense pain or the opposite, nerve stimulus deprivation; psychological it can be severe sudden trauma, shock or being unable to cope with sometimes even simple thing, like a total lack of hope or faith in the future. Sometimes it can be caused by chemical imbalances in the brain as well.
Now, cyberpsychosis (or CP) implies a state of psychosis which is set on by cyberware. On a side note, I personally think that in the world of 2077 even a normal psychosis would be booked as CP due to the omnipresent nature of cyberware, but I suppose the dystopian world of Night City would drive everyone mad. In the case of David, how easy would you be able to cope with what happened to his mom? Night City is full of combat veterans with PTSD, people have to survive on a 24/7 base even as normal workers. The stress levels are off the scale in anyone, I suppose.
However, back to CP; it is not unlikely that cyberware comes with unique challenges to the body. Where flesh is chrome swapped, so are a lot of nerves. This could be cause for either sensory deprivation or neurological trauma in the form of pain. The nervous system could well take a severe beating from either. Add to that combat stress, the taking of the occasional wound – and often not even taking a normal healing process; just maxdoc an airhypo, and the body takes another bit of physical trauma. Blood, gore, guys, death; losing loved ones, overstimulation by brain dances, a city that pounds you with information and advertisements, no vacations for the poor…
So, I basically see no reason why anybody would be spared a shot at cyberpsychosis, unless they are really rich or have continued Quantified Satori support.
Funfact, some people say that it would be nice if CDPR added cyberpsychosis mechanics to the game but they actually kinda did it. This is based on playing style for individual players. If you think about it and take a closer look at, you can see that for some players the beginning of the game is relatively "calm", while at a later stage of the game when the character is heavily improved, some players are "testing" new skills starting to killing random civilians and making a total demolition in city center for no reason.
i personaly belive that cyberpsychosis has 6 main factors
1: the amount of cybernetic augments an individual has
2: their natural fortitude
3: mental health/wellbeing
4: bodily health/physical trauma
5: the quality of the cybernetics
6: the quality of the implantation procedures
for 1: the more augments a person has the higher risk. we see this with maine, david, the lieutenant, and a few other individuals. to go further into that the more augments that deviate from the person's natural limitations the more severe it is again we see this with maine and david and several other individuals.
for 2: basicaly how well they can tolerate augmenttation it varies person to person and people like david and adam are prime examples
for 3: both maine and david experienced severe trauma in their life with david the death of his mother for maine his military background. the augmentations amplify that trauma and can heighten the effects of it
for 4: if the body or mind is damaged severely enough it can cause the cybernetics to kinda become unstable
for 5: corpos has never gone cyberpsycho and they being filthy rich have access to the best cybernetics
6: same as the first but instead with how invasive the proceedure is and access to anesthesia
all of these factors i would say have to line up just right on an individual basis for the mind to fully reject the cyberware and cyborg augmentations which causes cyberpsychosis. it isnt perfect but its the best i have IMO as to explain cyberpsychosis

I'm not sure about the mechanics of the game, but I mentioned this on another video: If David or Maine got some therapy/counseling, it would have worked itself out. I don't think the cyberware knows how to codify/quantify/process crisies of conscience, emotional/mental trauma, or any kind of existential/spiritual dilemma. To the human mind, these things are just as real as the physical world, so the cyberware makes it such. You have an artificial system literally screaming at you, "You have to do something about this!", and like substance abuse/addiction, you're falsely treating the symtoms instead of the cause-the vicious cycle of 'upping your meds' until your mind turns in on itself. In Adam Smasher's case, he's either a textbook sociapath, or he has Zen samurai mental focus. Even if his life is devoid of peace, it has certainty so his cyberware isn't telling him anything his mind is trying to suppress/ignore.
Cyberpsychosis would imply psychosis caused by cyberware/cybernetics/cyber-something. However an immunosuppressant (which is a real drug/treatment used in our real word) is, as the name applies, for suppressing the bodies immune system response.
So cyberpsychosis is a condition of the body that effects the mind. Nothing we don't already know but just want to add some clarification.
Adam Smasher is full borg thus negating the need for immunosuppressants or being susceptible to cyberpsychosis.
Also if we take a look at the book "The World of Cyberpunk 2077" in the Maelstrom section pg 160 par 1 sent 2, reads "Many of the gang's members are on the verge of developing cyberpyschosis, or already suffer from the condition". We don't really know more about the subject and what goes on behind the scenes of the Maelstrom but I feel its save to say most of them are running on the edge and they handle anyone that does cross over.
The book also explains the generations of cyberware, specifically stating that gen 3 cyberware is less likely to cause Cyberpsychosis. page 37, para 2, sent 2.
From the anime we can see David's cyberpsychosis accelerated by the amount of cyberware, the type of cyberware specifically the stress it puts on the body, his natural resistance to cyberpsychosis and his immediate mental state (ex when Faraday was antagonizing him via Lucy's "cyberphone" connection). Rebecca and Lucy also imply that him dialing back on the "chrome" and its usage will help prevent him from getting to the point of no return.
All that said it would seem Cyberpsychosis is just a disease that everyone has a propensity for. Someones outward appearance can mean very little. It share similarities with multiple sclerosis in a few ways. It also share some very vague similarities to alcoholism (disease) in other ways….bit of stretch there but I'm trying to find a disease that has its roots in the individuals actions so I'm sure there are better analogies there…
I also think there is different types of cyber psychosis for example for most of the game V could be discibed as experiencing a form of cyber psychosis and Adam Smasher definitely has almost no human parts left I think he has multiple control chips placed in him in lore then there is Maine witch is a almost textbook example of someone who's starting to show signs of it, Rebecca showed similar signs as Well but managed to keep more of her head
I thought him seeing the scientists sprouting guns was PTSD from him being tortured in the BD? That's also how the guy from the start goes insane because that BD guy says he did that to him, It was after David got captured he started to really lose it.
Cyberphycosis is basically your implants mess up your nervous system cause those implants slowly degrade your mind and body over time leading to you losing yourself, everyone that has implants is affected by this degradation but people who have less implants and less powerful implants aren't as affected and the degradation happens slower, the more you have and the higher grade they are the faster you succumb to the degradation which takes away your humanity other factors also play into it like mental disorders like PTSD for example or the over usage of medication and drugs that should make the degradation slower but using them to much and often can actually flip the coin on you and make the degradation happen even faster and you become addicted to the meds also Daeoms and other forms of viruses can lead to the degradation happing faster or those Daemons just straight up fry your brain and take over your body plus if you use your implants to often especially if those are military grade like Mains and David's it just f's up your body and mental state and makes the chances to get Cyberphycosis even higher then normal
it basically boils down to everyone can get it even if you have just one implant and even if those are cheap everyday ones if you have bad luck and it completely degrade your nerves and mentality you can become a Cyberphyco every person has a different body after all meaning some can handle it and others can't in the end no mater how much chrome you got you are just as affected by the degradation like that other guy who has a million, for you it just takes longer to get into a bad state if it can get into such a state before you die

Basically, all David needed was therapy. XD And all Maelstrom boosters have GOT to be cyber-psychos! Nobody does all that to themselves without being a little off.
Really true that mindset and therapy comes into it though. Biggest problem for most people, even in the videogame, is that they can't access proper healthcare or therapy to help with adjustment. While the drugs keep the symptoms of psychosis at bay, if you aren't getting the mental help, it won't happen.
Not all who suffer the psychosis go violent either – may not be connected to military cyberware. There was a cyberpsycho in a quest in-game who V can talk down (before he kills himself, anyway). Former military, planning violence but not yet over the Edge.
Most of all, I agree with some others in the comments who say it was because David, Maine and crew didn't slow down and adjust or repair what they had. Rebecca and others even suggest to just cool off and let the brain adjust. Even in the TTRPG, if a PC keeps chipping without any recovery of Humanity, it's a very quick descent. Humanity may be a stat, but I've always played it as representative too. Just packing on the chrome? Has to do a number on the brain.

Cyber psychosis is a type of dissociative disorder. This sounds similar to jokers bad day and deadpool. Both deadpool, joker and even rick Sanchez are all so self aware about their reality that their action seem insane. All info on cyberpsychosis seem to talk about their humanity and environmental factors. So its more philosophical based. Except that cyberpsychosis is caused by cyberware. In one of the comics on joker talk about something called hyper sanity. Which they illustrated an example.
Imagine a clock. Insanity is a broken clock, unable to conform and follow the crowd. Having little control over their reality. Sanity is a regular working clock. Self aware and able to conform. Hyper sanity is the person watching the clock. Aware of its existence on a more comprehensible scale.
So my theory is that “cyber”psychosis isnt actually an illness but a break through. The realization of the truth. And that can break a person. I imagine that the hallucinations are part in do to stress. But Is maybe because their suppressing so much about the situation that it just all piles up till they break.
As for deadpool well he breaks the fourth wall and can see into other realities. So Im guessing it isnt really a mental illness but a philosophical realization that would obviously cause so much stress that it makes people break.

I've heard a theory about V and their cyberpsychosis, but I unfortunately I cannot remember the details. While I remember is that after they got flatlined by Dex after slotting the Relic, it's been maintaining the brain so that Johnny can take over, which means it's likely also fighting against cyberpsychosis. Unless you're killing cops and bystanders on purpose, you're technically not going cyberpsycho as you're not going through any similar symptoms, no matter how much cyberware your V has. If I remember the theory correctly, it's also because V is so focused on having their own life back as they don't want it to be taken over by Johnny, you could say they're holding onto their humanity very tightly so they don't lose track on what their goals are.
Edit: You could also say that V is special like David, but as I said, their goal is stopping the Relic from erasing them.
Edit 2: I also just had a thought right now as I wrote the first edit. Since the Relic we took was an experimental version that wasn't meant to be sold and remain in-house, it could also be seen as a way to cure, or at the very least, stop the process of cyberpsychosis. Kind of.
it all comes to brains and nervous system. All cyberpsychos either have brain damage or nerves damage or both. even David is mentally unstable from the beginning (After mother's death), after he pushed his body through hell he started to lose his mind
