The WTH is an easter egg thing apparently, I didn't play the game it is referencing, so it freaked me out when I first saw it too. Hideo Kojima game if that answers your question. Edit: Also, in both playthroughs I did River brutally ran over multiple civilians in the drive to red peaks, so that person was really lucky.
